Multimodal MRI Features of Medulloblastoma with Extensive Nodularity and Activated Sonic Hedgehog: A Case Report

چکیده
: Medulloblastoma with extensive nodularity (MBEN) is a rare histological subtype of medulloblastomas in very young children. It characterized by an aggressive grape-like appearance and favorable outcomes. Herein, we report the case nine-month-old patient MBEN, associated activated sonic hedgehog (SHH), multimodal magnetic resonance imaging (MRI) features. Conventional MRI findings indicated dense homogeneous enhancement large unusual gyriform mass cerebellar vermis both hemispheres. The showed restricted diffusion on diffusion-weighted images, low concentrations taurine MR spectroscopy, relatively cerebral blood volume dynamic susceptibility contrast (DSC) perfusion image. underwent partial tumor resection, chemotherapy, autologous stem cell transplantation. After treatment, lesion rapidly regressed serial follow-up without recurrence.

A rare case of medulloblastoma with excessive nodularity: imagistic features
Medulloblastoma is the most common malignant tumor of childhood. Neuroimaging can play a role in the diagnosis of medulloblastoma, however atypical features do exist [2]. We report the case of a 1 year and 10month-old infant diagnosed with a medulloblastoma with what we term “excessive” nodularity based on neuroimaging features and confirmed by neuropathology. CT-scan (CT) and magnetic resonanc...

Medulloblastoma with extensive nodularity is a rare subtype of the most common malignant childhood brain tumor and has been associated with more favorable prognosis. The authors report the case of a 10-month-old girl with a posterior fossa tumor of excessive nodularity with decreased diffusivity on diffusion-weighted magnetic resonance imaging sequences and robust grape-like postgadolinium cont...
متن کاملMedulloblastoma With Extensive Nodularity: Tailored Therapy in a Low-resource Setting.
Treatment for medulloblastoma carries significant risks, particularly in resource-constrained settings. We report a case of a Mexican infant with desmoplastic/nodular medulloblastoma. Given the nature of her tumor, we developed a tailored regimen following subtotal resection to avoid both radiation therapy and the high-dose cisplatin therapy offered at most centers in the United States. The pat...
متن کاملMedulloblastoma with extensive nodularity: single photon emission CT study with iodine-123 metaiodobenzylguanidine.
We present the case of an infant with medulloblastoma with extensive nodularity, which had been called cerebellar neuroblastoma. MR imaging clearly revealed a nodular enhancement, which appeared as a grape-like lesion. Single photon emission CT revealed markedly high iodine-123 metaiodobenzylguanidine uptake in the enhancing tumor. Iodine-123 metaiodobenzylguanidine single photon emission CT ma...
متن کاملGenetic drivers of metastatic dissemination in sonic hedgehog medulloblastoma
Leptomeningeal dissemination (LMD), the metastatic spread of tumor cells via the cerebrospinal fluid to the brain and spinal cord, is an ominous prognostic sign for patients with the pediatric brain tumor medulloblastoma. The need to reduce the risk of LMD has driven the development of aggressive treatment regimens, which cause disabling neurotoxic side effects in long-term survivors. Transposo...
